Descriptive Summary:
This presentation will provide a brief overview and examples of the most common medication errors in the outpatient setting. The differences between errors of omission and commission, and strategies and best practices to prevent them, will be discussed. The role of interprofessional and patient communication in preventing errors will be addressed.
Learning Objectives:
Upon completion, participants will be able to:
- Differentiate between medication errors of commission and omission;
- List the most common system-based causes of medication errors in the outpatient setting;
- Identify medication classes/categories identified as high-alert medications by the Institute for Safe Medication Practices;
- Describe strategies to prevent the most common types of medication errors; and
- Explain the importance of communication to improve patient safety.
7:00 pm — Optimizing Opioid Safety for Pain Management: Preventing Errors Across the Healthcare Continuum

Descriptive Summary:
Prescription opioids, while effective for pain management, pose significant risks if not used appropriately. This presentation will focus on ways to prevent common errors in opioid prescribing, dispensing and administration that can have serious consequences for patient safety. The role of education and training in opioid safety for healthcare professionals at all levels to mitigate risks and enhance patient outcomes will be discussed.

Upon completion, participants will be able to:
- Identify common sources of errors in opioid prescribing, dispensing and administration;
- Discuss key strategies for preventing errors in opioid prescribing, dispensing and administration; and
- Highlight the role of education and training programs in improving healthcare professionals’ awareness of and competency in opioid safety.
8:00 pm — Managing Mishaps: Understanding Medication Errors in Diabetes Care
Presenter:
Nissa Mazzola, PharmD, CDCES
Descriptive Summary:
Medication errors are a worry for all patients, but those with diabetes often have even greater concerns given the fact that they are frequently prescribed multiple medications, which can potentially induce hypoglycemia. This presentation will focus on the prevalence of medication errors in the management of diabetes and strategies to mitigate these risks, including both individual patient practices and systemic approaches.
Learning Objectives:
Upon completion, participants will be able to:
- Discuss the prevalence and scope of medication errors in diabetes management, including their impact on patient outcomes and healthcare systems;
- Understand factors that contribute to medication errors in the treatment of diabetes, such as issues related to medication reconciliation, patient adherence and healthcare provider communication;
- Explain evidence-based strategies and best practices for preventing, detecting and mitigating medication errors in diabetes care, encompassing both patient-centered interventions and system-level initiatives; and
- Describe how interdisciplinary collaboration among healthcare professionals can help prevent medication errors in diabetes management through improved communication and coordinated care plans.

Members must have been a part-time or full-time employee for at least one year, work at least three-fifths of a regular workweek, and work for an employer who contributes to the 1199SEIU League Training and Upgrading Fund or the 1199SEIU Greater New York Education Fund.
City of New York Education, Child and Eldercare Fund Members
Pharmacists, Dietitians, Clinical Laboratory Practitioners and LPNs must have worked with NYC Health + Hospitals or a mayoral agency for at least one year.
Registered Nurse Training and Job Security Fund members
Members must have been a part-time or full-time employee for at least one year and work for an employer who contributes to the 1199SEIU/Hospital League Health Care Industry Registered Nurse Training and Job Security Fund.
